Your First Visit

Thank you for choosing Hampton Family Dentistry of Middletown for your dental needs.

We strive to provide a comfortable environment where you'll feel confident in the level of dental care you receive.

When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office.

For your convenience our patient forms are available online and to download. Feel free to print and fill out the forms and bring them with you to your first appointment. These documents require the use of the free Adobe Acrobat Reader. If it's not already installed on your computer, download it here.

If your first visit is for a dental emergency, only the tooth in question will be addressed. Depending on the diagnosis, an additional appointment with adequate time may be necessary.

Covid-19 Forms

Supplemental Health Questionnaire , is for every patient on every visit as a screening tool for as long as the COVID-19 remains a threat. This is because symptoms may develop between orthodontic appointments. This is for the protection of the doctor, staff and other patients/parents.

HEPA air filtration equipment

This office is protected by HEPA air filtration equipment to safely capture chairside aerosols and droplets for the health and safety of our patients! An extraoral dental suction unit is positioned chairside during any procedures that create aerosols and droplets, this helps protect our operatory form airborne contaminants.

Protocols that we have implemented for the protection of our patients/parents the doctor and staff for all who enter the office:
1. Covid-19 Health Questionnaire prior to appointment
2. Wearing of a face mask or covering
3. Temperatures are taken
4. Washing hands before being seated
5. Patient check in call from their car for social distancing

Insurance Options

Our office is committed to helping you maximize your insurance benefits.

Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid as per the contract agreement.

As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them. If your insurance covers orthodontic treatment, you will receive the benefit of reduced personal costs. Many insurance policies have a lifetime orthodontic benefit that is distinct from regular dental insurance. Insurance policies vary, so we will review your insurance to determine the appropriate course of action.

Once treatment has started, we will file your claims. To help us with your insurance filing, please complete the insurance questions on our New Patient Registration Forms. Hampton Family Dentistry accepts most insurance plans. Please have your insurance information accessible when scheduling your first appointment - that way we can verify your insurance benefits and minimize your out-of-pocket expense! If your plan requires a dentist referral, please obtain one before scheduling your complimentary consultation to avoid treatment delays. Please bring your insurance card and information to your consultation appointment.
